We thank you for your interest in being a part of ASH Networks' network of Contracted Clubs. Below are the materials you need to print, sign and return to ASH Networks to begin the contracting process. Please print the Return Checklist [PDF] to assure you have included all necessary documentation required by ASH Networks to complete the contracting process as quickly as possible.

All fitness clubs wishing to participate must successfully meet the Health and Safety Guidelines and Code of Conduct [PDF] requirements prior to contract execution.

Please remember to fill out all documents completely, include a copy of your liability insurance and send the following materials back to:
