ASH subsidiaries pay commissions using the following payment schedule:

What is the distinction between the education programs made available by Healthyroads Inc. and the benefit programs offered by ASH subsidiaries?
There is no risk associated with the health education programs provided by Healthyroads, Inc., and these programs are not regulated.

The California Department of Managed Health Care regulates the HMO programs offered by American Specialty Health Plans of California (ASH Plans).

The at-risk benefit programs offered by ASH Insurance are fully regulated by the Department of Insurance in the appropriate state agency for the state in which the programs are offered.

Is there a minimum group size for enrollment in the programs available through ASH subsidiaries?
American Specialty Health Insurance Company (ASH Insurance)
At-risk benefit programs: 51 or more eligible employees

American Specialty Health Networks, Inc. (ASH Networks)
ASO benefit programs: 1,000 or more eligible employees

American Specialty Health Plans Of California, Inc. (ASH Plans)
HMO benefit programs: 2 or more enrolled employee

Healthyroads, Inc.
Education programs: 2 or more enrolled employees

What are the underwriting requirements for the benefit programs?
Programs must be offered alongside an employer-sponsored group medical plan.
 
  • Each employee/dependent enrolled in the employer-sponsored medical plan must also be enrolled in the program.
  • The programs may not be offered on a voluntary basis.
  • The employer may elect to exclude all dependents and offer the rider to employees only (a 30 percent load applies).
  • Employer must contribute at least 75 percent of the employee price.
  • Effective dates are the first of the month.


Are voluntary programs available?
Individuals may purchase health education programs directly from Healthyroads, Inc. online at www.healthyroads.com. Benefit programs are not available on a voluntary basis.

How do I enroll a group?
In order to enroll a group, please send the following items to your account executive.
 
  • Group Application
  • Binder check for first month's premium/fee
  • Copy of the most recent bill from the employer-sponsored medical plan
  • Employee enrollment provided in one of the following ways:
    • Completed ASHIC/ASHP/Healthyroads enrollment form for each employee
    • Copies of the employer-sponsored medical plan enrollment form for each employee
    • Electronic file (contact your Account Executive for details)
  • For the broker/consultant (first group only):
    • Completed Producer Agreement
    • Copy of broker/firms license
    • W-9 form
    • Proof of Errors and Omissions insurance

How do I complete a Broker of Record Change?
In order to complete a Broker of Record change, please submit the following items to your Account Executive.
 
  • Completed producer agreement, copy of brokers/firms license, W-9 , and E & O (for first group only)
  • Broker of Record change cover letter for group (see sample [word])
