Payor Exceptions to ASH Guidelines

ASHA medical necessity determinations will be made in accordance with ASHA Clinical Practice Guidelines and Aetna Clinical Policy Bulletins (CPBs).  In the event that an ASHA Clinical Practice Guideline differs from an Aetna Clinical Policy Bulletin regarding the same procedure/protocol, the Aetna Clinical Policy Bulletin will be the controlling document for an Aetna member.
